Individual Supports

The Clubhouse provides a wide range of programs and services which assist members to live in the community independently and with dignity. However, for some members, to do this requires additional, more intensive and individualized support.

The Center’s Community Support staff works with members to develop independent living skills and assist them as they take on more and more of the management of their day-to-day lives. Because the CS staff works so closely with the units within the Clubhouse, there is a synergy – open communication and a sharing of information – that is hard to find in typical rehabilitation and case management structures. Community Support can include:

• Assisting members in obtaining and applying for resources and accessing services in the community such as Medicaid, food stamps, psychiatric services, medication supervision, oversight and other entitlements.
• Working with community based housing programs as well as with Independence Center Residential Program services to find housing choices that balance member supports with the opportunity for increased independence.
• Assisting members in developing daily living skills such as cleaning, grocery shopping, hygiene and scheduling.
• Providing emergency assistance and crisis intervention for members, including working with friends and family.
